Yellow Door social support services help you live independently and remain socially active.

We understand that it can become more difficult to get out and about as we age or if we are living with a disability.

Yellow Door can facilitate a range of social support services that are tailored to your social needs and preferences. This may involve getting out and about with a caregiver, visiting your favourite café or attending a sporting or community event. Or, it may be as simple as spending your time at home, chatting with a caregiver over a cup of tea.

To understand your preferences, a Yellow Door Care Coordinator will seek to understand your hobbies, interests, goals and culture before creating your care plan, which will include remaining socially connected in the ways that are important to you.

Social support services will often involve Yellow Door providing assisted transport, accompanying you and providing personal support along the way.

Services can include, but not limited to:

  • Seeing a movie or show
  • Attending a family get together or a community event
  • Going shopping
  • Going for a scenic drive
  • Starting a new hobby or refining a skill
  • Sharing a meal or cup of tea
  • Enjoying a game of cards
  • Baking and cooking at home
  • Visiting the local library

Our social support services can be paid for privately, through your Home Care Package or through your NDIS plan.
